Annual Health and Medical Record (AHMR)
The Annual Health and Medical Record (AHMR) is a vital document required for everyone in Scouting America. The AHMR is an annual requirement for all participants-both youth and adults – for everything from unit meetings to high adventure treks. Completing this form fully helps to keep everyone in the program healthy and safe. The benefits of an annual health check by your personal healthcare professional are numerous.
Benefits can include:
- Early detection of health issues which may be identified sooner which can lead to better health outcomes. Healthcare professionals can screen for conditions like elevated cholesterol, high blood pressure, vision changes and so many others. Quick identification can help provide treatment before they can become serious.
- Preventative care and review of vaccination status which can help to reduce future issues and illnesses.
- Establishing a baseline of your current health data is important so that future comparisons can be made and monitored.
- Patient-Doctor relationships are strengthened. The opportunity for you to connect personally with your healthcare professional is so important.
- Health coaching tailored to your specific needs and health history. Preventing health issues is a key component and meeting with your doctor at least annually begins that process.
The AHMR is divided into several sections. When to complete each of the sections depends upon which activity you will be participating.
Parts A & B
Everyone- adults and youth must complete Parts A and B for all events.
It should be reviewed at least annually and revised anytime there are changes in your medical information.
Part C
Part C is required for all events lasting longer than 72 hours. Some councils may also require completion for some activities or events. It is a requirement for treks at all four of the national high adventure bases.
A licensed healthcare professional (M.D., D.O., Physician Assistant, or Nurse Practitioner) must conduct the pre-participation physical examination and complete the Part C form. They will review your health history, complete the form and discuss your fitness for the activities you will be participating in.
High Adventure Risk Advisory
A specific High Adventure Risk Advisory has been developed by all four of the Scouting America High Adventure Bases. Some councils that offer high adventure or other activities may also have a risk advisory.
The Risk Advisory outlines some of the high adventure program experiences and how certain medical conditions may affect you during those strenuous activities. It is best to review the information prior to your pre-participation examination. Bring the risk advisory to your appointment with your healthcare professional and discuss your specific activities so you can be best prepared for your adventure.
